School Based System Of Care Liaison (Full Time, Hybrid, Durham County, North Carolina Based) (Education)



The School Based System of Care Liaison fosters the collaboration of schools, public agencies, law enforcement and private providers that offer mental health, developmental disabilities and substance abuse treatment as well as other human service and governmental agencies such as vocational rehabilitation, housing, juvenile justice system, health care, child welfare, and self-help agencies.

This is a full-time hybrid opportunity. The selected candidate must be available to report onsite to the Alliance Office (Morrisville, North Carolina) to attend business meetings as needed. They will also be expected to travel throughout the Durham County area to serve Alliance members as needed. 

Responsibilities & Duties

Provide Education and Technical Assistance to Community Partners

  • Educate and provide technical assistance in a consultative capacity to community partners regarding the availability of local services, methods for accessing services, the provider network, mental health reform and the System of Care
  • Provide information, education and consultation to public and private entities that routinely interact with individuals with special health care needs or other high-risk populations
  • Develop resources that can be utilized by enrollees of the MCO and public school system
  • Work with provider agencies to ensure the occurrence of and fidelity to Child and Family Team process and provide technical assistance as needed.  Work in conjunction with QM to evaluate fidelity and effectiveness

Foster Collaboration of Families, Schools, Public Agencies, Law Enforcement, and Private Providers

  • Represent the organization in the community, in regard to special assistance and programs offered by the organization and increase community awareness of initiatives and School Based Mental Health  
  • Facilitate the process to connect members to provider community that includes educating about consumer choice, person-center-planning, cultural competence and Care Review Team and Recovery Team processes
  • Foster participation and involvement of youth and families at all levels of the System of Care, includes youth and family representation at each local collaborative and provides and supports leadership opportunities for youth and families
  • Work with other specialists and care coordinators on cross-disability issues (i.e. Housing, Employment, Homelessness, Financial, Community Collaborative, Care Review, etc.)
  • Make referrals and aid in the early identification of individuals with special health care needs and other life complex issues
  • Consult with treatment teams, community collaboratives and other groups serving enrollees of the public school system
  • Develop school inclusion as system of care process to support School Based Mental Health
  • Serve as staff to the local community collaborative(s), recruit and maintain membership that includes public school system, family members, consumers, youth, child-serving agencies and community partners, and facilitate the decision-making process regarding flexible funds

Address System Level Impacts

  • Promote a collective impact approach to system level challenges and fragmented care (serving as the backbone)
  • Work with community agencies in identifying and responding to community needs, network adequacy and service accessibility needs
  • Participate in interagency efforts in support of the Behavioral Health and Physical Health systems
  • Promote and forge relationships with physical and behavioral health providers across the Alliance catchment area to work collaboratively to reduce barriers and improve access to health care
  • Assist in the development of the organization Needs/Gap Assessment
  • Develop in-depth knowledge of various community systems and provide consultation to Alliance clinical departments, and public school system regarding available resources and barriers for individuals who are involved with various community systems, i.e., Department of Social Services, Department of Public Safety, etc.

Develop goals

  • Develop goals and support School Based Team in achieving successful outcomes related to Key Performance Indicators

Travel 

  • Travel between Alliance offices, attending meetings on behalf of Alliance, participating in Alliance and/or school sponsored events may be required
  • Travel to meet with members, providers, stakeholders, attend court hearings etc. is required

Minimum Requirements

Education & Experience

Bachelor's degree in Psychology, Social Work, Counseling, Public Policy, or related field from an accredited college or university with a minimum of three (3) years combined applicable clinical experience and administrative experience.  

Experience in any of the following areas would be valuable for this employee:  Public speaking, presentation development, cultural awareness and advocacy.

Employment for this position is contingent upon a satisfactory background and MVR (Motor Vehicle Registration) check, which will be performed after acceptance of an offer of employment and prior to the employee's start date. 

Salary Range

$25.75 - $32.83/Hourly 

Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity.
