Schedule: Sunday, Monday, Tuesday, alternating Wednesdays | 10:00 AM - 10:00 PM
Job Type: Full-Time (Set Schedule)
Job Summary:

Ahwatukee Post Acute is seeking a dedicated and experienced Charge/Admission Nurse to join our skilled nursing team. This position is essential to overseeing unit operations, managing patient admissions, and ensuring continuity of care during assigned shifts. The ideal candidate will demonstrate strong clinical judgment, leadership ability, and a commitment to high-quality resident care.
Responsibilities:

• Serve as the charge nurse during scheduled shifts, supervising nursing staff and ensuring delivery of appropriate care.
• Coordinate and manage patient admissions, including clinical assessments, documentation, and communication with families and interdisciplinary teams.
• Monitor resident conditions, administer medications, and provide direct nursing care as needed.
• Ensure compliance with state and federal regulations, facility policies, and standards of practice.
• Communicate effectively with residents, families, staff, and outside providers.
• Document all care and assessments accurately in the medical record.
• Support ongoing quality improvement and infection control initiatives.

Qualifications:

• Current Arizona RN or LPN license in good standing.
• Minimum of 1 year experience in a skilled nursing facility or long-term care setting preferred.
• Prior experience as a charge nurse or with admissions strongly preferred.
• Strong leadership, organizational, and clinical skills.
• Ability to work the assigned schedule consistently.
• Knowledge of PointClickCare or similar EMR systems is a plus.
