Health Learning Facilitator (Finance)



Minimum Qualifications: This position requires a bachelor's degree in nursing, Nutrition, Dietetics, Health Education, Public Health or other related health field with at least one year of experience in a professional training position; or an associate's degree in nursing and licensure as a Registered Nurse with at least three years of experience in a professional training position. A master's degree in public health education or any of the above-listed degree fields is preferred. Certain grants and programs have specific education and/or certification requirements specific to the subject matter of the program area (i.e. Certified Diabetes Educator -CDE; CPR; etc.).

Preferred Qualifications: Experienced certified pump trainer on all insulin pumps. Certified Diabetes device trainer for minimum of 3 years

  • Collaborates with providers to develop care plans, evaluates patients, and develops individualized education and training. Provides a month of evaluation, adjustment, and support following pump starts. Documents education, training, and updates in the patient record. Communicates all activities with patients' providers.

  • Develops and/or maintains guidance documents for providers, support staff, and patients regarding operation of diabetes technologies and data uploading. Maintains pump training room and sample pump and sensor supplies. Disposes expired supplies or labels them "For Training Only." Provides real-time support for in-clinic device uploading.

  • Monitors and responds to in-basket tasks and patient communications regarding diabetes technologies, excluding prescriptions / orders and insurance concerns.

  • Contributes to the Diabetes Technology Research Program. Completes study-related device training as needed and assists with uploading device data during research visits. Assists with study recruitment.
  • \n
  • Participates in Diabetes Center staff meetings and Diabetes Care Conferences to plan, evaluate, and improve our diabetes services.
  • \n
  • Completes billing documentation, maintains billing database, and submits pump start invoices for reimbursement.
