Case Manager (R&D)

The Case Manager assists individuals in designing and implementing goal-oriented, individualized strategies to access services and supports. Responsibilities include assessment, planning, linkage, advocacy, coordination, and monitoring to ensure consumers gain access to needed health care, dental care, housing, employment, education, financial assistance, and natural supports. This role incorporates LifeWays mission, vision, and values into all decision-making processes.
Essential Functions

Develop and implement person-centered service plans that support consumer goals and medical needs.

Coordinate and monitor access to health care, housing, financial, employment, educational, and social services.

Provide crisis intervention, discharge planning, and care coordination, including for court-involved consumers.

Advocate for consumers and collaborate with families, providers, and community resources to ensure quality care.

Support independence, recovery, and community inclusion through culturally competent and trauma-informed practices.
