Assessment & Referral Mental Health Professional (Personal Services)

As an Assessment & Referral Mental Health Professional you will work in coordination with the Essentia Health Providers as a member of the treatment team, to coordinate the mental health care needs for Essentia patients. To consult with patients in crisis or with a need for mental health services, and refer them to the appropriate level of care. To collaborate with the other primary care disciplines to ensure the mental health needs of the patients are being met.

Ideal candidates will have a passion for providing a high level of care to all outpatient patients, have strong organizational skills, enjoy collaborating with others, and have an interest in continuing to develop competencies as a clinician.

We are a team of mental health professionals that specialize in psychiatric evaluations, clinical social work, substance use disorders and family therapy. We are committed to helping patients in our community who are experiencing personal, emotional, marriage, family or psychological problems. Founded in 1991, we offer an intensive outpatient substance use disorder program, psychological testing, and behavioral health and medication management services.

Job Duties:

Conducts visits with Essentia patients as requested by Essentia clinical staff to do any of the following:

Brief suicide risk assessment
Brief crisis intervention
Conduct a psychosocial assessment to determine the needs of patients
Brief psychotherapy/supportive counseling
Discussion of other NAL/MH services needed
Referral to other NAL services at the Baxter Office and other community social services as indicated

Conducts curbside consultation with Essentia providers, which are brief in-person clinical consultation as needed
Maintains legible and organized patient electronic health records in the Essentia EMR, in accordance with policy of NAL & Essentia, and requirements of the State of Minnesota. Concurrent documentation is mandatory.
Has an understanding of and refers to NAL programs and coordinates care.
Attend regularly scheduled Essentia staff meetings.
Acts as a conduit to NAL to expedite and prioritize the coordination of mental health services.
Follows policies and procedures of Nystrom & Associates (NAL) & Essentia.
Other duties assigned.


Qualifications:

Licensed Mental Health Professional in Minnesota (LICSW, LMFT, LPCC, or LP)
Applicants with a dual licensure of a LADC are preferred but not required
Master's or doctoral degree in a behavioral health field
Unrestricted License
