Position Summary: The Hospice Social Worker (MSW) provides emotional support, counseling, and advocacy for patients and families. This role helps individuals cope with the physical, emotional, social, and spiritual challenges associated with terminal illness, while ensuring care aligns with patient and family goals.
Key Responsibilities:
Conduct comprehensive psychosocial assessments of patients and families.
Provide counseling, emotional support, and crisis intervention.
Assist patients and families with advance care planning and end-of-life decision-making.
Educate families on community resources, benefits, and support services.
Advocate for patient rights and help navigate complex care needs.
Participate as a core member of the interdisciplinary hospice team.
Maintain timely and accurate documentation per hospice regulatory standards.
Qualifications:
Master of Social Work (MSW) required.
Current Missouri licensure.
Previous experience in hospice, palliative care, or medical social work preferred.
Strong communication, counseling, and advocacy skills.
Compassionate, patient-centered approach with cultural sensitivity.
