Social Services/Discharge Planner (Personal Services)

Location: Midtown Oaks Post Acute - Sacramento, CA
Pay: $30-$40/hr DOE

Midtown Oaks Post Acute is seeking a Social Services Director to support our residents, families, and care team through all stages of the post-acute journey. We're looking for a compassionate and organized professional who thrives in a fast-paced environment and is committed to resident-centered care.

What You'll Do:

• Lead discharge planning to ensure safe, smooth transitions
• Assist residents and families with Medi-Cal applications and other benefits
• Serve as a liaison between residents, families, and the interdisciplinary team
• Advocate for resident needs and psychosocial well-being
• Participate in care conferences and collaborate on individualized care plans
• Help residents adjust to facility life and promote emotional support

What We're Looking For:

• Experience in social services or discharge planning in a healthcare setting
• Strong communication, documentation, and problem-solving skills
• Familiarity with Medi-Cal processes and community resources
• A team player who is reliable, approachable, and solution-focused
• Bachelor's degree in Social Work or related field preferred
